Missed Your Mounjaro or Ozempic Dose? What to Actually Do
A missed weekly injection isn't an emergency, but the right move depends on how many days late you are. The actual rule, why restarting after a long gap needs care, and how to keep your India supply from causing this in the first place.

If you're within 4 days of your usual injection day, take the dose as soon as you remember and continue your normal weekly schedule from there. If it's been longer than 4-5 days, skip the missed dose entirely and wait for your next scheduled day - don't double up. This is the standard guidance across GLP-1 drugs (semaglutide and tirzepatide alike), though your prescriber's specific instruction always takes priority over a general rule.
At a Glance
- Within 4 days late: take it now, resume normal weekly schedule
- More than 4-5 days late: skip it, wait for the next scheduled dose
- Never double up to 'catch up' - this raises side-effect risk without added benefit
- A gap of 4+ weeks may need a dose restart at a lower level - check with your doctor
- Most missed doses in India trace back to supply gaps, not forgetfulness - plan ahead
The Rule, and Why It Exists
These medications are dosed weekly because their effect builds up gradually in your system over days, not because a single injection "runs out" sharply on day 7. A few days' delay barely changes blood levels. Doubling up, on the other hand, spikes the dose and meaningfully raises nausea and GI side-effect risk without giving you any extra weight-loss benefit - the drug doesn't work faster at a higher momentary dose, it works on sustained weekly exposure.
If It's Been Longer Than a Few Weeks
This is where it gets more specific to you. If you've missed 4+ consecutive weeks, many prescribers restart at a lower dose rather than resuming where you left off, because side-effect tolerance (particularly nausea) can partially reset during a long gap - restarting at your previous higher dose can hit harder than expected. This is exactly the kind of decision that needs your actual doctor, not a general guide - call before your next injection rather than guessing.

Start Your Journey →Why This Happens So Often in India Specifically
Supply gaps are the leading real-world cause of missed doses here, more than forgetfulness - stockouts at specific pharmacies, price swings between brands, and the newer generic semaglutide options creating confusion about which product is which. See our generic semaglutide guide for the current brand landscape, and consider keeping one dose's buffer stock rather than refilling exactly on schedule, given how often supply has been inconsistent through 2025-2026.
What to Watch For After Restarting
Nausea, fatigue, and GI symptoms are more likely to reappear noticeably after a gap of several weeks, similar to what you felt when first starting - see the full pattern in our side effects management guide. Keep protein and hydration priorities in place exactly as you did when you first started; the Mounjaro diet guide applies just as much on a restart as on day one.
Preventing the Next Missed Dose
- Set a recurring weekly phone reminder tied to a fixed day, not "whenever I remember"
- Order your next month's supply the week you start the current one, not when it runs out
- Keep the pen refrigerated correctly - a pen left out of the cold chain too long may need to be discarded, which is its own common cause of an unplanned gap
